There are a range of forms required to ensure that we can provide medication to learners. Please view the forms below. We can support you in school to print and complete any medical form.
•Form 1 (if you want school to administer short-term non-prescribed medication)
•Form 2(if you want school to administer short-term prescribed medication)
•Form 3(if you want school to issue long-term prescribed medication)
•Form 4 (if you want school to issue long-term ‘as required ‘prescribed medication)
•Form 5 (If you want your child to self-administer e.g. inhalers)
Any forms completed at home should be returned to the school office.
